This document is a Price Proposal Template (Attachment D) for the Veterans Health Administration (VHA) National Teleradiology Program (NTP) Physician Services. It provides a uniform format for Offerors to submit proposed pricing for the requirement.

• The template is organized by five (5) ordering periods, each with specific CLINs (Contract Line Items).

• Offerors must enter their fully burdened, proposed unit prices only in the yellow-highlighted cells.

• The template must be submitted in its original Excel format. PDF or other non-editable formats may be rejected.

• The file should be named "Name_Price Schedule_Date".

This document is critical for bidders to understand the required pricing structure and submission format. It outlines the specific services (Digital X-Rays, CT Scans, MR Scans, US Scans) and quantities for each ordering period, as well as the mandatory format for submitting pricing. Questions & Answers / Clarifications

This document contains responses to questions submitted by potential bidders regarding the solicitation for teleradiology services.

• NAICS Code: The Government will retain NAICS code 621111 (Offices of Physicians, except Mental Health Specialists) for this acquisition, as it is for physician professional services, not the operation of a diagnostic imaging center. The size standard for 621111 will apply.
• SLA Fee: A 3% Service Level Agreement (SLA) fee is to be paid quarterly by the Contractor to SAC, as detailed on pages 49-51 of the solicitation.
• CT Volume Breakdown: For CLIN 0002 (CT services), past volumes indicate approximately 30% of CTs were STAT CT AP and 15% were STAT CTAs. Further breakdown for CTAP and CTA AP was approximately 1.3% of all STAT CTs and 8% of all STAT CTAs.
• Test Result Notification: The Contractor's mechanisms must support VA's obligations under VHA Directive 1088 for communicating test results to providers and patients.
• Radiologist Qualifications: The Government will not revise language in PWS section 5.1.3(1)(a) regarding radiologists with actions against their license. The current language ensures all contract radiologists meet VA's credentialing and quality standards.

Amendment / Modification

This amendment (0002) to solicitation 36C10G26R0015 modifies the original solicitation in the following ways:

• Provides answers to vendor questions via Attachment E.

• Adds new CLINs (0005 to 4005) to the price schedule to include a Service Level Agreement (SLA) fee.

• Provides a revised Attachment D (Proposal Price Template).

• Extends the proposal receipt date.

• Attachment D (Proposal Price Template) is revised.

• The price schedule is updated with new CLINs for SLA fees.

• The proposal receipt date is extended.

• The proposal receipt date has been extended to April 6, 2026, at 9:00 am EDT.

• Offerors must acknowledge receipt of this amendment. If an offer has already been submitted, changes can be made via letter or electronic communication, provided it references the solicitation and amendment and is received before the new deadline.
