Statement of Work (SOW) / Performance Work Statement (PWS)

To provide waste pick-up service for the LAWTON INDIAN HOSPITAL.

• Provide daily waste pick-up service, six days a week (Monday through Saturday), excluding holidays.

• The contract period is July 1, 2026, through June 30, 2027, with a 12-month base period and four 12-month option periods.

• Provide three (3) eight-yard metal trash containers, including maintenance or replacement as needed.

• Submit a proposed schedule of tasks upon award, detailing planned service days.

• Submit requests for payment.

• Submit status reports quarterly to the Project Officer.

• Submit any other reports required by the Federal Government.

• Submit original invoices to Division of Financial Management, Oklahoma City Area Indian Health Service, 701 Market Center Drive, Oklahoma City, Oklahoma 73114.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It is used by offerors to self-certify that their enterprise meets the definition of an "Indian Economic Enterprise" as defined by the Buy Indian Act and HHSAR 326.601. This certification is required in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Self-Certification: Offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Period: The enterprise must meet the definition throughout the offer period, at the time of contract award, and during the full contract performance period.
• Notification Requirement: Contractors must immediately notify the Contracting Officer if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility and successful offerors must be registered with the System of Award Management (SAM).
• Legal Consequences: False or misleading information is a violation of law and punishable under 18 U.S.C. 1001 and 31 U.S.C. 3729-3731, 18 U.S.C. 287.
• Required Fields: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of IEE Firm
  • Business Name
  • Certifying Signature
  • UEI Number
  • Print Name

This form is critical for any bidder seeking to participate in opportunities that are set-aside or awarded under the Buy Indian Act. Bidders must accurately complete this form to self-certify their eligibility. Failure to meet the definition or provide accurate information can lead to disqualification or penalties. Successful offerors must maintain this status throughout the contract performance period.

Sources Sought Notice

This notice is issued by the Indian Health Service (IHS) to identify Indian Small Business Economic Enterprises (ISBEEs) capable of performing a requirement for three (3) eight-yard metal trash containers, including maintenance or replacement as needed. The IHS intends to use the Buy Indian Act authority to give preference to ISBEEs for this requirement, if authorized and practicable.

• Provide three (3) eight-yard metal trash containers.

• Provide maintenance or replacement of containers as necessary.

• Upon award, submit a proposed schedule of tasks, including a flow chart detailing when services will be performed and planned days of the week for services.

• Interested parties must submit a capability statement demonstrating their ability to meet the requirement.

• Responses must include specific company information (Name, Socioeconomic Status, SAM UEI, contact details, DUNS, Cage Code, Tax ID, GSA Schedule/procurement vehicle, SAM registration status).

• Submit the attached Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form.

• Responses must be emailed to Mr. Jarrod Prince (jarrod.prince@ihs.gov) or Mr. Carroll Delaune (carroll.delaune@ihs.gov) no later than 1:00 PM CST on May 19, 2026.

• The subject line of the email must include the notice number: IHS1523580.

• Maximum submission length is five (5) pages.

• This notice specifically seeks ISBEEs. Other small businesses are also encouraged to respond.

• Vendors must be registered in SAM.gov.

• This is a Sources Sought notice, not a solicitation for proposals.

• Generic capability statements are insufficient; responses must directly demonstrate capability.

• No proprietary, classified, confidential, or sensitive information should be included.

• The government reserves the right to use non-proprietary technical information in future solicitations.

• Responses will not be returned and will not be accepted after the due date.

• The government is not obligated to award a contract based on this notice.
