This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." • Self-Certification: Offerors must certify whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Eligibility Period: The enterprise must meet the definition throughout the offer period, contract award, and the full contract performance period.
• Notification Requirement: Contractors must immediately notify the Contracting Officer if they no longer meet eligibility requirements.
• Documentation: Contracting Officers may require documentation of eligibility and awards may be subject to protest.
• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Penalties: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products or services for trash services at the Lawton Indian Hospital. It is a Small Business Set Aside.

• Provide trash services for the Lawton Indian Hospital.

• Must be able to comply with pick-up daily, six days a week (Monday-Saturday), excluding holidays.

• Must provide three (3) eight-yard metal trash containers and maintain or replace them as necessary.

• Upon award, the contractor must submit a proposed schedule of tasks.

• Contract Type: Firm Fixed Price Purchase Order.

• Period of Performance: Base year 07/01/2026 – 06/30/2027, plus four option periods.

• Location: Lawton Indian Hospital, 1515 NE Lawrie Tatum Road, Lawton, Oklahoma.

• Submission Deadline: 06/19/2026, 01:00 p.m. CDT.

• Submission Method: Electronic submission via email to Jarrod Prince (jarrod.prince@ihs.gov).

• Evaluation Factors: Technical (all or none requirement), Past Performance, and Price.

• Award will be made to the responsible Offeror whose offer is most advantageous to the Government.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide waste pick-up service for the Lawton Indian Hospital.

• Provide daily waste pick-up service, six days a week (Monday through Saturday), excluding holidays.

• Provide three (3) eight-yard metal trash containers, including maintenance or replacement as needed.

• Submit a proposed schedule of tasks upon award, detailing planned service days.

• Period of Performance: July 1, 2026, through June 30, 2027.

• Location: Lawton Indian Hospital, 1515 NE Lawrie Tatum Road, Lawton, Oklahoma.

• Submit requests for payment and quarterly status reports to the Project Officer.

• Submit original invoices to Division of Financial Management, Oklahoma City Area Indian Health Service, 701 Market Center Drive, Oklahoma City, Oklahoma 73114.

• Federal legal holidays observed under the contract include: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Juneteenth Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.
