Performance Work Statement (PWS)

To provide Urology Surgical Navigation System & Support services at the 88th Medical Group, Wright-Patterson Air Force Base, Ohio.

• Provide UroNavigation Fusion System for MRI Biopsies.

• Provide Ultrasound System for Da Vinci Robotic Nephrectomies.

• Provide Cyber TM Thulium Laser for Holmium Laser Enucleation of the Prostate (HoLEPs).

• Equipment is designed for clear, real-time ultrasound-guided biopsy images and visualization of lesions.

• Services are performed at the facility at least 1-2 times per month.

• Contractor technicians must check in with SYMPLR and wear green paper scrubs.

• Technicians must have at least 12 months of experience and certifications to operate the specified equipment.

• Technicians provide intra-operative equipment monitoring under the direction of the Urology provider.

• Equipment must be delivered and set up the day before or the day of surgery to ensure availability.

• Equipment must be cleared through the Medical Group's Medical Equipment Repair Center (MERC) before patient use.

• Coverage is provided Monday-Friday, 7:00 am - 4:30 pm, excluding Federal Holidays.

• Contractor is responsible for transport, maintenance, repair, and calibration of their equipment.

• Government will monitor contractor performance through scheduled and unscheduled reviews by the Quality Assurance Evaluator (QAE).

• Services are to be performed at Wright-Patterson Air Force Base (WPAFB), Ohio.

• Normal operating hours are Monday-Friday, 7:00 am - 4:30 pm.

• Contractor must maintain all licenses, certifications, and insurance.

• Fee schedule for services will be established, with potential annual increases capped at 6% and requiring approval.

• A minimum of twenty-four hours' notice is required for scheduled procedure cancellations. Less than 24 hours' notice for cancellation will result in the contractor being paid the set-up/break-down fee.

• Contractor must provide a minimum of two weeks' notice for any dates of non-availability.

• Invoicing must be accepted via Government Purchase Card (GPC).

• Limitation of Liability: Contractor is not responsible for loss or damage unless attributable to their negligence or willful misconduct, patient care, or supplies/equipment provided by WPAFB.
