Statement of Work (SOW)

To ensure all U.S. Coast Guard (USCG) Training Center (TRACEN) Petaluma Fire Department personnel meet medical and physical standards required for duty, and to identify and document any medical conditions that may affect job performance or safety.

• Provide annual and pre-placement physical examinations for designated fire department personnel.

• All examinations must comply with the latest edition of NFPA 1582: Standard on Comprehensive Occupational Medical Program for Fire Departments.

• Clinical laboratory services must be performed by a U.S. Department of Health and Human Services (DHHS) licensed laboratory.

• Base Period: August 1, 2026 to July 31, 2027, with four one-year option periods through July 31, 2031.

• Primary place of performance will be the Contractor's medical facility.

• Coast Guard personnel may travel up to a 20-mile radius to the Contractor's facility.

• Perform a physical examination of each employee, documenting results on the NFPA Physical Exam Form 1582 or equivalent.
