Scope of Work
The contractor shall fabricate high-quality fixed dental prosthetics according to dental laboratory prescription specifications. This includes providing all transportation, shipping, delivery, labor, supplies, supervision, quality assurance, and materials. Key requirements include:
- Prosthetic Types: Metal crowns/bridges, porcelain-fused-to-metal, ceramic restorations, metal partial denture frameworks, and full/partial dentures.
- Integration: Ability to integrate combined treatment needs for patients requiring simultaneous fixed and implant-supported prostheses.
- Workflow: Support for conventional, digital, or combined workflows, including a secured web portal for digital submissions.
- Delivery: Transportation and delivery services directly to and from the Roseburg and Eugene clinics.
- Materials & Fabrication: Use of FDA-approved materials, with all fabrication and assembly occurring within the United States.
