Performance Work Statement (PWS)

To provide non-personnel services for equipment maintenance of the VersaTREK 240 Blood Culture System.

• Perform scheduled and unscheduled maintenance and calibration inspections on one (1) VersaTREK 240 Blood Culture System and one (1) Computer system.

• Use Original Equipment Manufacturer (OEM) standards and only OEM parts.

• Scheduled maintenance includes operational checks, calibration verification, cleaning, lubrication, electrical checks, parts replacement (if necessary), visual inspection, and consultation with the operator.

• Unscheduled maintenance is for repairing malfunctioning or inoperable equipment and is unlimited in number.

• Provide all necessary personnel, equipment, supplies, facilities, transportation, tools, and materials.

• Furnish and install software updates and equipment modifications at no additional cost.

• Provide 24-hour toll-free telephone assistance for equipment issues.

• Equipment must maintain a monthly percentage uptime of 95%.

• No more than 2 valid customer complaints per quarter.

• Service reports detailing all services rendered must be submitted upon completion of each service call.

• Period of Performance: One (1) Base Year of 12 months, three (3) 12-month option years, and one (1) 6-month extension.

• Place of Performance: 55th Medical Group, 2501 Capehart Road, Offutt AFB, Nebraska 68113-2160.

• Contractor personnel must be trained by the OEM or qualified medical equipment service school.

• Use only new OEM or factory refurbished replacement parts.

• If equipment is removed for service, provide similar equipment at no additional cost.

• Comply with all applicable safety standards (Federal Regulations, OSHA, Air Force safety standards).

• Contractor personnel must identify themselves as such to avoid creating an impression of being Government officials.

• Adhere to specific response times for scheduled (3 working days in advance) and unscheduled (no later than 2 workdays after request) maintenance.

• Contractor shall provide a contract manager responsible for performance.
