Wage Determination

This document is a Register of Wage Determinations under the Service Contract Act, specifically Wage Determination No. 2015-5005, Revision No. 27, with the last revision date of December 3, 2025. It establishes minimum wage rates and fringe benefits for various occupations in Iowa (Counties of Harrison, Mills, Pottawattamie) and Nebraska (Counties of Cass, Douglas, Sarpy, Saunders, Washington).

• Vacation: Paid vacation is provided based on years of service: 2 weeks after 1 year, 3 weeks after 8 years, 4 weeks after 15 years, and 5 weeks after 20 years.
• Holidays: A minimum of eleven paid holidays per year are required, including New Year's Day, Martin Luther King Jr.'s Birthday, Independence Day, etc. Contractors may substitute holidays with prior communication.
• Hazardous Pay Differentials: Specific differentials apply for work involving ordnance, explosives, and incendiary materials (8% for high hazard, 4% for low hazard).
• Uniform Allowance: If uniforms are required, the contractor must provide them at no cost to the employee or reimburse the employee for the cost. Reimbursement for cleaning and maintenance is also required ($3.35 per week), unless uniforms are "wash and wear."

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide non-personnel services for equipment maintenance for the VersaTREK 240 Blood Culture System.

• Provide all necessary personnel, equipment, supplies, and services for maintenance of the VersaTREK 240 Blood Culture System.

• Perform scheduled and unscheduled maintenance in accordance with Original Equipment Manufacturer (OEM) standards.

• Use only new OEM or factory refurbished replacement parts.

• Provide telephone assistance 24 hours a day for equipment support.

• Furnish and install all software updates and equipment modifications provided by the manufacturer at no additional cost.

• Maintain a 95% monthly uptime for all systems.

• No more than 2 valid customer complaints per quarter.

• Scheduled maintenance to be coordinated at least three working days in advance.

• Unscheduled maintenance response time: no later than 2 workdays after service request.

• Period of Performance: One (1) Base Year of 12 months, three (3) 12-month option years, and one (1) 6-month extension.

• Contractor personnel must be trained by the OEM or a qualified medical equipment service school.
