Statement of Work (SOW) / Performance Work Statement (PWS)

To provide telecommunication services, including Voice POTS and fax lines, Primary Rate Interface (PRI), and Dedicated Internet Access (DIA) with 250 Mbps or higher bandwidth, to the Dzilth-na-o-dith-hle Health Center (DZHC) in Bloomfield, New Mexico.

• Provide and maintain fix wireless and fiber circuit connectivity between DZHC and NNMC.

• Supply POTs and fax lines, PRI DIOD, and Internet DIA features.

• Ensure continuous availability of DIA, Voice POTs, and PRI DIOD services.

• Maintain a 99.9% uptime service level agreement for circuit availability.

• Respond promptly to accidental circuit outages caused by indirect telecommunication personnel.

• Conduct circuit studies upon request for slow transmission rates.

• Period of Performance: 07/01/2026 – 06/30/2027

• Place of Performance: Dzilth-Na-O-Dith-Hle Health Center (DZHC), Bloomfield, New Mexico.

• The contractor must protect equipment from damage or theft and prevent voice landline, PRI, and internet service disruption.

• Adherence to all protocols, patient confidentiality, and record keeping consistent with HIPM Privacy Act and JCAHO regulations.

• Ensure circuits are available at all times; in case of power or network outage, contact the local power company for restoration or restore any cut copper or fiber lines.

• Onsite work requires access to the Main IDF closet.

• Contractor must notify MIS and obtain authorization before dispatch.

• Contractor must wear a company-provided identification badge.

• Adherence to Health Safety Rules when entering the facility.

• Must sign in with Security and receive a visitor badge.

• Compliance with HIPAA regulations as implemented by the U.S. Department of Health and Human Services.

Sources Sought Notice

This notice is a Sources Sought for Indian Economic Enterprise (IEE) and Indian Small Business Economic Enterprise (ISBEE) vendors capable of providing telecommunication services to the Dzilth-Na-O-Dith-Hle Health Center (DZHC). The services include Voice POTs, fax lines, Primary Rate Interface (PRI), and Dedicated Internet Access (DIA) of 250 Mbps or higher bandwidth. This is not a solicitation, but an RFI to identify potential vendors.

• Provide and maintain telecommunication services including:

  • Primary Rate Interface (PRI) lines
  • Voice POTs and fax lines
  • Dedicated Internet Access (DIA) with >= 250 Mbps bandwidth

• Ensure 99.9% service level agreement for circuit availability.

• Secure and ensure wireless and fiber circuit connectivity between DZHC and NNMC.

• Provide standard or custom reports on monthly usage (Claims Submissions, Claims Status Checks, Eligibility Checks, Insurance Searches).

• Adhere to security, privacy (HIPAA), and safety protocols.

• Period of Performance: July 1, 2026, through December 31, 2026.

• This is a Sources Sought notice, not a solicitation. Responses will be used to refine acquisition strategy.
