  • Factor 2: Past Performance: Evaluates relevance, quality, timeliness, and customer satisfaction of up to three (3) recent and relevant contracts.
  • Factor 3: Price: Evaluated for fairness and reasonableness.

This document is a Quality Assurance Surveillance Plan (QASP) for the "PRESCRIPTION SAFETY GLASSES" contract. Its purpose is to outline the methods and procedures the Government will use to ensure the contractor meets the services identified in the Statement of Work (SOW). It also aims to hold the contractor accountable for quality control and accurate invoicing.

• Scope of Work: Provides comfortable, quality Rx safety eyewear for WCF personnel, including onsite optician visits for comprehensive optical services, a variety of frame styles and lenses (single, bi-focal, tri-focal, progressive, tinted, polycarbonate), and professional consultation.
• Contract Type: Blanket Purchase Agreement (BPA).
• Roles & Responsibilities: Defines the roles of the Contracting Officer (CO) and Contracting Officer's Representative (COR) in contract administration and technical monitoring.
• Contractor Responsibilities: Includes issuing safety glasses, ensuring proper fit, meeting ANSI Z87.1 standards, providing a point of contact, submitting accurate invoices within 14 days of delivery, and being available during scheduled shifts.

This QASP provides potential bidders with a clear understanding of the government's expectations for quality, performance monitoring, and contractor responsibilities. It highlights the standards the contractor must meet, the surveillance methods the government will employ, and the procedures for addressing non-conformance, all of which are critical for developing a compliant and competitive proposal.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide Bureau of Engraving and Printing (BEP) / Western Currency Facility (WCF) personnel with comfortable, quality Prescription (Rx) safety eyewear. This includes providing comprehensive optical services such as onsite fitting and eye examinations (excluded).

• Provide Rx safety eyewear in accordance with employee prescriptions.

• Offer a wide variety of frames (styles, colors, single vision, bi-focal, tri-focal, progressive) and lenses (polycarbonate, tinted, gray, transition).

• Ensure eyewear meets ANSI Z87.1 standards.

• Provide professional consultation on eyewear type and fit for eye hazard protection.

• Onsite services will be provided during day, evening, and midnight shifts.

• Eyewear must meet ANSI Z87.1 standards.

• Vendor must provide a point of contact within 1 week of award.

• Invoices must be accurate and submitted within 14 days of delivery.

• Vendor must comply with WCF Environmental, Health, and Safety (EHS) Policies and Procedures.

• Contract period: One (1) calendar year from award, with four (4) annual renewal options (FY 2026 - FY 2030).

• Place of Performance: 9000 Blue Mound Road, Fort Worth, TX 76131.

• Key Personnel: ABO/NCLE Certified Optician and a Licensed Optician with at least two years of experience.

• Frame Cost Limit: $50.00 per pair (employees may pay the difference for frames exceeding this limit).

• Employees receive one pair of safety glasses per prescription; police and facilities employees may receive a second pair or photochromatic lenses.

• Security: Access to the facility requires a REAL-ID compliant picture ID. Background investigations are required for contractor personnel.

• Contractor employees must wear identification badges and identify themselves as contractors.
