• CLINs: 0001 (Base Period), 1001 (Option Year 1), 2001 (Option Year 2), 3001 (Option Year 3), 4001 (Option Year 4).

Performance Work Statement (PWS)

The objective is to obtain reliable, performance-based laundry services for the Western Oregon Service Unit to ensure clean linens and related textile items are available for patient care and staff operations.

The Contractor shall provide all personnel, supervision, management, transportation, equipment, supplies, and resources necessary to perform laundry services. This includes picking up soiled laundry, processing it, returning clean laundry, and providing service records. Laundry items may include linens, towels, gowns, lab coats, scrubs, blankets, and other patient-care or staff textile items.

Estimated weekly quantities are provided for planning purposes only and are not guaranteed. Key items include:

• Isolation gowns, large: 145

• Scrub pants/shirts (various sizes): 30-60 per size

• Patient gowns, large: 60

• Surgical towels: 240

• Surgical towel inventory maintenance: 5 billing units Total estimated items: ~820 per week, ~42,640 per year.

• Pickup and Delivery: Once per week, during normal business hours. AQL: No more than one late/missed pickup/delivery per quarter, corrected within one business day.

• Cleanliness and Usability: Items returned clean, dry, odor-free, and usable. AQL: Defective items reprocessed/replaced within two business days.

• Packaging: Clean items protected from contamination during transport.

• Inventory Accountability: Service records provided with each delivery, noting exceptions. AQL: Complete records sufficient for invoice review.

• Re-performance: Rejected laundry corrected within two business days or by next delivery.

• Continuity of Service: Maintain arrangements to continue performance during disruptions. AQL: Timely notice and corrective action within one business day.

• Infection Control and Safety: Comply with applicable health, safety, and environmental requirements.

• Customer Service: Respond to Government concerns by the next business day.

• Period of Performance: Base Period: July 19, 2026 – July 18, 2027, with four Option Years through July 18, 2031.

• Place of Performance: Pickup and delivery at Western Oregon Service Unit, Chemawa Health Center, Salem, Oregon. Laundry processing at Contractor's or approved subcontractor's facility.

• Contractor personnel must comply with facility access, security, and safety requirements.

• Contractor is responsible for all performance, including subcontractors.

• No personal services are authorized.

• Contingency procedures for service disruptions are required.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form."
