• Materials & Coatings: Specifies materials (CR39, Poly, Trivex, 1.56, 1.6, 1.67, 1.74) and coatings (Clear, Transition, Xtractive, Polarized, Drivewear, Crizal AR coatings, Blue Capture).
• Performance Periods: Data is provided for the following periods:

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products and services, specifically for eyeglasses and lenses for the Wewoka Indian Health Center. It constitutes the only solicitation, and a separate written solicitation will not be issued. The acquisition is an ISBEE set-aside.

• Provision of optical lenses and frames.

• Includes a variety of mid-range priced metal and zyl frames (unisex, women's, men's, children's), including non-allergen materials, wide width, and titanium frames.

• Provision of table top displays for clips and demo lenses.

• Contractor to update frame inventory every six months, removing discontinued frames and replacing them.

• All frames include a 1-year warranty for remounting/inserting lenses.

• Lenses to include plastic, polycarbonate, high index, and glass materials, with safety thickness and scratch-resistant coatings (1-year warranty).

• Tinted lenses with solid and gradient options.

• Specific lens brands preferred: Essilor or Varilux.

• No extra charges for high sphere, cylinder, or oversize blanks.

• Order forms and pre-paid shipping labels to be provided by the contractor.

• Completed spectacles to be returned within 7-10 days of the order date.

• Contractor to provide fabrication of eyeglasses services.

• A representative to visit annually (or virtually) to update staff on new products and policies.

• Performance standards include timely delivery (within 10 calendar days), low contractor error rates (95%), and low product defect rates (95%).

• Contract Type: Firm Fixed Price 5 Year Blanket Purchase Agreement.

• Period of Performance: May 08, 2026 – May 07, 2031.

• Evaluation Factors: Technical (Guarantee, Past Performance), and Price.

• Technical Evaluation includes: Guarantee for frames and lenses breakage/scratches (1-year replacement), and Past Performance (reviewed via CPARS and other sources).

• Price Evaluation: Based on a completed pricing schedule, evaluated by multiplying quantities by price for a cumulative total.

• Offerors must be registered in the System for Award Management (SAM).

• ISBEE set-aside.

• Requires completion of the Indian Economic Enterprise Representation Form.

• Incorporates FAR and HHSAR provisions and clauses by reference.

• Vendors must submit: Completed Price Schedule (Excel spreadsheet), Completed IEE Form, and Guarantee and Redo/Refund Information.

• No fax quotations accepted.
