This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Criteria: The form requires the offeror to represent whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act. This definition must be met at the time of offer, contract award, and throughout the contract performance period.
• Required Information: Offerors must provide:
  • Name of Federally Recognized Tribal Entity
  • Name of the 51% owner of IEE Firm
  • Business Name
  • DUNS Number
  • Certifying Signature
  • Print Name
• Legal Implications: The form warns that false or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Additional Requirements: Successful offerors must also be registered with the System of Award Management (SAM).

This form is critical for any bidder seeking to participate in opportunities designated under the Buy Indian Act. Bidders must accurately complete this representation to certify their eligibility. Failure to meet the criteria or provide accurate information can lead to disqualification or penalties. It also highlights the need for SAM registration.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide secured shredding services for the Indian Health Services, Whiteriver Service Unit, including the Whiteriver Indian Hospital and Cibecue Health Center.

• Provide secured, lockable shredding bins (95 or 65-gallon capacity, or standard consoles) at designated locations.

• Collect and shred confidential documents, including paper, folders, and thin paper pamphlets.

• Services to be performed monthly during normal business hours (8:00 am to 5:00 pm, Monday-Friday, excluding federal holidays), unless otherwise approved by the COR.

• Shredding must be crosscut, rendering documents unreadable. Strip shredding is unacceptable.

• Contractor is responsible for providing all necessary equipment, including bins, consoles, and shredding machinery.

• Contractor personnel must be uniformed, identifiable, trained, licensed, and insured.

• Contractor shall not inspect, view, or disclose any material being shredded.

• Shredding must be crosscut and ensure all destructed documents are unreadable.

• Contractor shall provide Certificates of Destruction (COD) upon completion, indicating the number of bins serviced.

• Base Year: June 1, 2026 – May 31, 2027

• Four (4) Option Years: June 1, 2027 – May 31, 2031

• Locations include Whiteriver Indian Hospital campus, Administration Building, Specialty Services Building, Garrett Building, and Cibecue Health Center.

• Contractor must maintain confidentiality in accordance with federal and state laws and industry best practices.

• Contractor personnel must sign a confidentiality statement.

• Contractor shall retain ownership of containers and consoles and maintain them in good working order.

• Contractor must provide five (5) keys for locking containers at each location in Whiteriver, AZ, and for the Cibecue Health Center POC.

• Pricing proposal must detail all related costs, fees, and capabilities, including fees per bin, console, pound, and for special shredding requests.
