Statement of Work (SOW) / Performance Work Statement (PWS)

To renovate the 4th floor of Building 5 at the Jackson VA Medical Center to create space for EHRM training and command center operations, supporting the EHRM deployment.

• Provide all labor, materials, supplies, parts, and equipment for the renovation.

• Install new walls, finish flooring, and finish ceilings.

• Install new lighting, power, and fire alarm systems.

• Install new plumbing fixtures.

• Provide and install new HVAC units and systems specifically for the 4th floor.

• Remove existing lab services, including special exhaust, electrical distribution, and laboratory gas systems.

• Work scheduling must minimize facility impact; after-hours and weekend work may be required.

• Adhere to VA provided plans and specifications.

• Work must be performed in accordance with relevant safety standards, codes, and manufacturer's/industry standards.

• Contractor must ensure all equipment functions in conformance with VA safety standards, FDA, OSHA, JC, and other applicable national standards.

• Contractor must provide a one (1) year warranty on all equipment and services.

• The period of performance is 365 days from the Notice to Proceed.

• Work is to be performed at the Jackson VA Medical Center.

• Contractor personnel must wear Vendor ID Badges.

• Contractor personnel are responsible for their own parking and any associated violations.

• Work hours are Monday through Friday, excluding National Holidays.

• A valid hot work permit is required for welding, cutting, bracing, and soldering.

• A functional fire extinguisher must be available in the work area.

• The work area must be cleaned before leaving the premises.

• Contractor must provide a point of contact for purchase order administration.

• Foreman and personnel require specific OSHA training and current TB tests.

• Contractor must comply with all applicable records management laws and regulations, including NARA policies and the Privacy Act.

• Contractor shall not create or maintain records containing non-public GVSMVAMC information not tied to the contract.

• Contractor shall not retain, use, sell, or disseminate deliverable information covered by the Privacy Act or FOIA exemptions.

• The GVSMVAMC owns all data and records produced under the contract.

• Contractor employees handling records must complete specified records management training.

• Subcontractor requirements must be incorporated into subcontracts.
