Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work (SOW) outlines the requirements for fire door repairs to address deficiencies identified in a recent inspection at the Department of Veterans Affairs Central California Health Care System (VACCHCS) in Fresno, CA.

• Perform repairs on identified fire doors based on a list of deficiencies.

• Comply with the latest published versions of NFPA 90A, NFPA 80, and NFPA 105 standards.

• Provide a 30-day warranty on all components, equipment, and labor.

• Specific deficiencies include excessive clearance, missing/damaged door closers, missing/damaged smoke seals, missing filler plates, doors that do not latch, and doors requiring relabeling.

• Two doors require relabeling by Intertek.

• Three doors require replacement due to missing labels, major damage, or exposed cores.

• Place of Performance: VACCHCS, 2615 E. Clinton Ave, Fresno, CA 93703-2223.

• Period of Performance: To be completed 60 days after Notice to Proceed.

• Contractor must provide all necessary tools, equipment, and personal protective equipment.

• Disposal of waste and debris is at the contractor's expense.

• A competent individual experienced in this type of work must be appointed for quality control.

• All work must be coordinated with the COR and commence only after approval.

• Normal work hours are Monday-Friday, 8:00 am – 4:30 pm, excluding Federal Holidays. Work outside these hours requires authorization.

• Utility shutdown requests must be submitted in writing at least 21 days in advance.

• Adherence to applicable codes and standards, including HCSM, OSHA, Lock-out-Tag-out, NFPA, etc.

• Smoking is prohibited on VACCHCS grounds and inside buildings.

• Contractor must provide one designated point of contact.

• Minimum PPE requirements include hard hat, safety glasses, reflective vest, and safety shoes.

• Adherence to OSHA 10 CFR 1910 and 1926.

• A safety plan must be submitted within 15 days of contract award.

• Safety Data Sheets for all chemicals used must be submitted and kept on-site.

• Compliance with EPA standards for disposal of waste and chemicals.

• Vendor must notify VA police upon arrival and obtain a temporary badge, which must be worn visibly.

• Contractor employees must have visible identification.

• Vendor is responsible for safeguarding their equipment and property on government premises.

• OI&T Security and accreditation requirements do not apply to this service contract.

This is a Sources Sought Announcement for Fire Door Repairs at the VA Central California Health Care System (VACCHCS). No solicitation is currently available. The Department of Veterans Affairs (VA) Network Contracting Office (NCO 21) is gathering information from contractors capable of providing these services.
