Statement of Work (SOW)

To repair an underground water pipe leak near Building 125 at the Michael E. DeBakey VA Medical Center in Houston, Texas.

• General: Provide all labor, materials, tools, and equipment to repair a 2", 6", and 8" PVC C900 water pipe. The work will be phased and may require work after normal hours and on weekends. Coordination with security and the COR is mandatory.

• Site Work: Includes demolition of existing concrete, excavation to locate and access the leak, repair or replacement of damaged piping, and installation of new concrete. Specifics include:

  • Excavation near Bldg 125 sidewalk/parking lot, Boiler Plant road, and Spinal Cord parking entrance.
  • Repair/replacement of damaged pipe sections with new materials equivalent to or better than existing, requiring COR approval.
  • Installation of concrete thrust blocks for new pipe sections/fittings at direction changes.
  • Backfilling excavation, compacting soil, and installing new concrete (minimum 6" thickness) with rebar and plastic sheeting.
  • Repair of any damage to existing concrete parking areas.

• Phases: The work is divided into three phases: initial examination and material procurement, water outage for pipe repairs (handled by VA Plumbing Shop), and completion of repairs and concrete work.

• All new materials must be equivalent to or better than existing.

• New piping must withstand campus water supply system temperatures, pressures, and flows.

• COR approval is required for materials, components, attachment methods, and functional testing.

• Concrete work must meet specific thickness, rebar, and mix requirements.

• All materials and components must be tested and fully functional upon completion.

• Duration: Work shall be completed within 30 calendar days after receipt of Notice to Proceed.

• Submittals: Quality Control Plan prior to work; manufacturer spec sheets after examination; as-built shop drawings at completion. All in electronic .pdf format.

• Coordination: Contractor must coordinate with the COR for excavation, work methods, schedules, shut down requirements, hot permits, safety requirements, traffic barricades, and utility shutdowns.

• Safety: Compliance with OSHA, Infection Control Risk Assessment, Hot Work Permits, and Confined Space Permits as needed.

• Work Hours: Normal hours are 7 am-4:30 pm, Monday-Friday. Work outside these hours requires COR approval.

• Parking & Storage: Contractor parking is restricted to authorized areas. Storage space is limited and must be coordinated with the COR.

• Debris Removal: Daily removal of waste and debris off-site.

• Warranties: One-year warranty from project acceptance.
