Statement of Work (SOW) / Performance Work Statement (PWS)

To replace existing streetlight pole concrete bases and associated electrical components at the St. Cloud VA Health Care System.

• Replace twenty-six (26) existing 30'-0" high streetlight pole concrete bases with new, higher concrete bases (2'-0" diameter x 8'-6").

• Replace approximately 7415 feet of electrical wiring between light poles.

• Replace light pole head feeds, fuse holders, and fuses at (26) locations.

• Perform paint touch-up on approximately (31) light poles, including rust repair and treatment.

• Provide all transportation, labor, personnel, supervision, equipment, tools, materials, and necessary offsite disposal.

• Keep as many streetlights operational as possible during the process.

• Vendor must provide submittals for all products prior to purchasing materials for VA review.

• New concrete bases must meet 5,000 psi compressive strength after 14 days, be air-entrained, class SI, with a maximum slump of 3 inches.

• Existing light pole conductors must be inspected; if in good condition, repull; if a concern, contact VA COR.

• The street light circuit must be operational each night.

• Test each light for proper operation upon completion.

• Place of Performance: St. Cloud VA Health Care System, 4801 Veterans Drive, St. Cloud, MN 56303.

• Period of Performance: 90 calendar days from notice to proceed (70 days field work, 20 days administrative period).

• On-site services during normal business hours (8:00 am - 4:30 pm, Monday-Friday), excluding Federal Holidays, unless coordinated with COR.

• Gray Market Prevention: No remanufacturers or gray market items are acceptable. Used parts are not permitted for repair or replacement.

• Safety: Vendor is solely responsible for compliance with all federal, state, and local laws, including OSHA standards. An Occupational Safety and Health Plan is required.

• Security: Contractor and sub-contracted personnel must wear visible Contractor Badges issued by Facilities Management. All personnel must check in with FM.

• Damage to Property: Vendor must prevent damage to Government equipment and property and report any damage to the COR within 24 hours.

• MS4 Compliance: No washdown or cleanout of cement/concrete trucks or used equipment on campus. Unused concrete, wash water, and debris must not enter the storm sewer system.

• Phasing: Work must be phased per set of streetlight poles on the same circuit to allow for proper lockout/tagout procedures.

• Submittals: Vendor to provide product submittals for VA review within 5 business days.

• Notice: Provide a minimum of 24-hour notice for any scheduled changes.

This document is a market research notice (Sources Sought) for the replacement of existing streetlight pole concrete bases at the St. Cloud VA Health Care System. The government is seeking information to identify qualified firms capable of performing this work and to determine the appropriate procurement method. This is NOT a request for quotes or proposals.
