Bidders must have their clients complete this questionnaire. The completed PPQ will be submitted with the proposal as part of Volume I – Factor 2 – Past Performance. The government will use this information to evaluate the bidder's past performance and assess performance risk. Clients are encouraged to submit the questionnaire directly to the offeror, who will then submit it to IHS with their proposal. The government reserves the right to verify all information provided.
