This document is a standard government form (FAR 52.225-2) used to collect information regarding the Buy American Act. Offerors must complete this certificate to indicate the origin of end products and components proposed for acquisition.

• Certification of Domestic End Products: Offerors must certify that each end product is domestic, except for those listed in paragraph (b).
• Foreign End Products: A section to list any foreign end products, their country of origin, and whether they exceed 55% domestic content (if applicable and not COTS).
• Domestic End Products with Critical Components: A section to list domestic end products that contain a critical component.
• Definitions: Key terms such as "commercially available off-the-shelf (COTS) item," "critical component," "domestic end product," "end product," and "foreign end product" are defined in the "Buy American-Supplies" clause.

Completion of this certificate is mandatory for offers. Bidders must accurately identify the origin and domestic content of their proposed products to ensure compliance with the Buy American Act. Failure to properly complete this form may impact the evaluation of the offer. The government will evaluate offers in accordance with FAR Part 25.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for refrigeration equipment for the Department of Veterans Affairs Prescott Healthcare System. The VA seeks a contractor to provide Top Freezer Refrigerators and Under-Counter Refrigerator/Freezers, specified as "brand name or equal." The equipment must meet or exceed the physical, functional, and performance requirements outlined in the Statement of Work (SOW).

• CLIN 0001: 40 units of Whirlpool Model WRTX7419SW or equivalent top freezer refrigerator.

  • Must be top freezer/bottom refrigerator design.
  • Refrigerator capacity: min 14 cu. ft.
  • Freezer capacity: min 5 cu. ft.
  • Total capacity: min 19 cu. ft.
  • ADA compliant.
  • Internal lighting.
  • Min. 2 adjustable glass shelves, 3 clear adjustable door bins, 2 clear crispers.
  • ENERGY STAR certified.
  • Electrical: 115 Volt / 60 Hz.

• CLIN 0002: 2 units of Summit Model ALRF48 or equivalent built-in under-counter refrigerator/freezer.

  • ADA Compliant.
  • Capacity: min 2.68 cu. ft.
  • Internal lighting.
  • Min. 3 removable door racks, 3 adjustable glass shelves, 1 slide drawer.
  • Max height: ≤ 32 inches.

• Delivery: FOB Destination. Shipping costs must be included in the unit price. A phased delivery schedule is desired but not mandatory.

• Contract Type: Firm-Fixed-Price.

• Period of Performance: Begins on the date of award. Deliverables must be delivered within 3 months After Receipt of Order (ARO).

• Response Date/Time: March 4, 2026, 10:00 AM Pacific Time.

• Questions Due: February 27, 2026, 10:00 AM Pacific Time. Submit questions via email only.

• Evaluation: Offers will be evaluated based on price, with the most advantageous offer to the Government being selected.

• Submission: Offers must be submitted by the response date. All offerors must be registered in www.sam.gov.

• Set-Aside: This solicitation is a total small business set-aside.

• NAICS Code: 333415 (Size Standard: 1,250 Employees).

• Buy American: Offerors must complete and return the Buy American Certificate (FAR 52.225-2) if offering non-domestic end-products.

• This announcement constitutes the only solicitation; a written solicitation will not be issued.

• Offerors must demonstrate that their "or equal" offers meet all salient characteristics. Failure to do so will result in a non-responsive offer.
