Eligibility & Facility Requirements

• Facilities must possess current Center for Medicare and Medicaid Services (CMS) certification (Medicare and/or Medicaid) and a state nursing home license.
• A minimum of a three-star overall rating or a two-star overall rating with a four-star or greater quality measures score on the CMS Nursing Home Compare website is required, or eligibility for a waiver.
• Must have bed capacity to ensure ability to take referrals.

Submission & Evaluation

• The solicitation is open and continuous for one year from June 18, 2024. Interested parties are invited to submit proposals for consideration during this period.
• Quotes must include the solicitation number, quoter's information, technical description, warranty terms, price, discount terms, "Remit to" address, completed representations and certifications (FAR 52.212-3), and acknowledgment of amendments.
• Evaluation Factors: Technical or Quality, Past Performance, Veterans Involvement, Price, and Geographical Location.
