This solicitation is for the procurement of DSI High Density Supply Storage Racking and Accessories for the Lebanon VA Medical Center (LVAMC) Surgery Center. The LVAMC currently uses the DSI Modumax system and requires additional units to accommodate the expansion of surgical services. The requirement is for "Brand Name or Equal" items.

• Items:

  • Modumax Starter Single Unit, CTS+ Width, with 10 Volkren-CTS+ Shelves and Dust Cover (Quantity: 4)
  • Modumax Adder Single Unit, CTS+ Width, with 10 Volkren-CTS Shelves and Dust Cover (Quantity: 9)
  • Modumax CTS+ Starter w/10 4BC-CTS+ and 1 Sterileshelf (Quantity: 1)
  • Modumax CTS+ Adder w/10 4BC-CTS+ and 1 Sterileshelf (Quantity: 3)
  • Assembly and Installation services (Quantity: 1)

• Salient Characteristics: Specific dimensions, material (tooled aluminum anodized tubing), load capacity (2000lbs), temperature resistance (-40°C to 140°C), warranty (Lifetime), and compatibility with the existing DSI Modumax system are detailed.

• Scope of Work: Includes providing, delivering, installing, and testing all items. The contractor is responsible for missing parts, ensuring OEM and VA specifications are met, and connecting with the existing system without disjointed framing.

• Installation: Must adhere to manufacturer specifications and safety standards. Contractor must remove all shipping materials and clean up debris. Responsibility for any damage due to contractor error is noted.

• Delivery: Within 30 days ARO (After Receipt of Order).

• Performance Period: Delivery within 30 days from award.

• Evaluation: Lowest Priced Technically Acceptable (LPTA) basis. Evaluation factors are Technical Specifications and Price.

• Responsiveness: Offerors must quote all items; failure to do so will result in ineligibility for award.
