Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ #: 36C25926Q0225) for new NP 12 Mattresses for the Eastern Colorado Healthcare System (ECHCS) located in Aurora, CO. The government anticipates awarding a firm-fixed-price contract.

• Product: NP 12 Mattresses, 36" x 80", without zippers.

• Quantity: 30 units.

• Make/Model Equivalent: Sizewize Envy G Series Non Powered Pressure Redistribution Mattress.

• Technical Specifications: For Med Surg use, 350 lbs weight capacity, 36 inches width, 80 inches length, designed to prevent pressure ulcers, with zoned layers, 4-way stretch top cover, non-skid polyurethane bottom, halogen-free fire barrier, 12-chamber pressure mapping, and no zippers.

• Delivery: Within 30 days of award to ECHCS, Aurora, CO 80045. Delivery must be in original sealed containers marked with PO and contract number.

• Documentation: 2 binders and 1 digital copy of manufacturer recommended maintenance and operation materials.

• Warranty: All manufacturer warranties on parts and labor, including travel and shipping costs for repairs, must be provided.

• Incorporated Provisions/Clauses: Effective through FAC 2025-06.

• NAICS Code: 337910 (Mattress Manufacturing), size standard 1,000 employees.

• Set-Aside: 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB). SDVOSB status must be verified in SBA's SBA Small Business Certification (SBS) at time of quote submission and award.

• OEM Requirement: Offerors must be an OEM, authorized dealer, distributor, or reseller, verified by OEM documentation. Gray market or remanufactured items are not acceptable.

• Attachments: FAR 52.225-2 Buy American Certificate and VAAR 852.219-76 VA Notice of Limitations on Subcontracting are required.

• Submission Volumes: Quotes must include Volume I (Technical Capability), Volume II (Price), Volume III (FAR 25 Provision), and Volume IV (VAAR 852.219-76).
