Amendment / Modification

This amendment to RFQ 36C26226Q0415 provides the following updates:

• Clarified Delivery Distribution:
  • (4) CD burners are to be delivered to the main facility at 650 E. Indian School Rd., Phoenix, AZ. The facility will handle internal distribution.
  • (2) CD burners are to be delivered to the 32nd Street Clinic at 400 N. 32nd St., Phoenix, AZ.
  • The contractor is responsible for configuring CD burners and training staff at all three listed sites.
• Corrected Product Service Code (PSC): The PSC has been corrected from the previous code 7E20 to 5836.

• Delivery distribution instructions have been clarified.
• Product Service Code (PSC) has been updated.

Written offers with spec-sheets are still required via email to israel.garcia2@va.gov no later than Friday, March 06, 2026, by 12:00 PM PST. No other changes to the terms and conditions of the solicitation have been made.

Solicitation

This solicitation is for the procurement of six (6) patient record CD burners and associated software for the Phoenix VA Healthcare System. The goal is to replace aging equipment and upgrade capabilities for burning patient images onto CDs for Community Care and ROI requests. This is a firm-fixed-price requirement.

• Six (6) fully automatic disc publishing systems (CD burners) with PC.

• Six (6) CD burner software licenses.

• Installation and application training.

• Equipment must meet specific hardware attributes (Windows 11, Intel i5 or better processor, 16GB RAM, 512GB SSD, 19-24 inch monitor, keyboard, mouse) and software attributes (DICOM import from VISTA Imaging and AGFA PACS, image compression for CD/DVD, simultaneous burning of at least 4 disks, customizable disk printing, web portal access, existing Enterprise Risk Assessment by VHA).

• Contractor shall provide at least a 1-year warranty on equipment and software.

• Contractor shall provide at least 2 virtual refresher trainings within the warranty period.

• Contract Type: Firm-Fixed-Price.

• Delivery: Within 90 Days After Receipt of Order (ARO).

• Warranty: Minimum 1 year.

• Questions Due: March 3, 2026, by 12:00 PM PST.

• Offers Due: March 3, 2025, by 12:00 PM PST.

• Submission Method: Email to Israel.Garcia2@va.gov in PDF format, no larger than 10MB.

• Evaluation: Offers will be evaluated for Acceptability (meeting Salient Characteristics and SOW) and then Lowest Price. Acceptability is the primary factor.

• This acquisition is a 100% Small Business Set-Aside in the open market.

• Offerors must be an OEM, authorized dealer, authorized distributor, or authorized reseller, verified by an authorization letter from the OEM.

• The solicitation incorporates FAR clauses by reference, including those related to commercial products and services, small business concerns, and electronic submission of payment requests.

• Specific locations for delivery are listed in the SOW.

This is a combined synopsis/solicitation for commercial products and services, issued as a Request for Quotation (RFQ) in accordance with FAR Part 12.6. The Department of Veterans Affairs, Network Contracting Office 22, is seeking to procure 6 Patient Record CD Burners and associated software for the Phoenix VA Healthcare System.

• Salient characteristics, Statement of Work (SOW), and attachment clauses/provisions must be adhered to.

• Delivery required no later than 90 days after receipt of order (ARO).
