This document is a Sole Source Justification for a firm-fixed-price procurement of AvaSure Software Licenses and support for the AvaSure TeleSitter Solution. The AvaSure TeleSitter Solution provides continuous remote video monitoring of patients to prevent falls and other adverse events.

• Procurement of 14 AvaSure software licenses and support (AVA-E-0104-1Y-R).

• Support for the currently installed AvaSure System at Columbia VAMC.

• This is a brand name justification for a sole source procurement.

• The statutory authority cited is FAR 12.102(b) Simplified Procedures for Certain Commercial Items, specifically 41 U.S.C. § 1901.

• The effective date of the justification is 12/30/2025.

• The solicitation will be publicized on SAM.gov as Unrestricted on the Open Market.

• A fair and reasonable price determination will be made by comparison to the Independent Government Cost Estimate, formulated using market research of historical pricing.

• Competition is restricted because only one responsible source (the OEM and its certified SDVOSB, Sierra7) can provide the proprietary software licenses and support for the existing AvaSure equipment.

• Recompeting the entire TeleSitter requirement would result in substantial duplication of cost.

• This requirement is for an IT commercial product and is exempt from restrictions outlined in FAR Part 25.

• Continued surveillance will monitor the industry for new entrants to the market.

• The equipment team will re-evaluate facility needs as the equipment nears the end of its life expectancy to maximize competition.

This solicitation is for the renewal of the AvaSure TeleSitter Software Support for the Harry S. Truman Memorial Veterans Hospital. The procurement aims to provide trained technicians for continuous remote patient monitoring via video to prevent falls and other adverse events. It includes software licenses for mobile tele-sitter devices requiring updates to MS Server 2016 or 2019.

• AvaSure Software - Standard for 14 devices, including AvaNET Silver Support, ORNA reporting tool, multi-language announcements, clinical program materials, and eLearning Modules.

• Software updates throughout the contract period.

• Extended Hardware Warranty for 14 Phase 1 devices for 2 years.

• The solution must be compatible with MS Server 2016 or 2019.

• Contract Type: Firm Fixed Price, FOB Destination.

• Period of Performance: Base Period (12 months) with four (4) Option Periods (each 12 months).

• Estimated Value: $47 Million (This appears to be an estimate for the total contract value).

• Offers are due by June 8, 2026, 11:00 am CDT.

• Technical questions are due by June 4, 2026, 11:00 am CT.

• Offers must be submitted in two parts: Priced Response and Non-Priced Response.

• Evaluation Factors: Technical Capability, Past Performance, and Price. Technical and Past Performance are significantly more important than Price.

• Past performance must be within the last 3 years and relevant to AvaSure TeleSitter support.

• This acquisition is for Total Full and Open Competition.

• It is a Brand Name Only procurement; vendors must be authorized distributors and submit an Authorized Distributor Letter from the OEM.

• Offerors must be registered in SAM and, if applicable, verified in SBA Small Business Search.

• The solicitation incorporates FAR clauses by reference, including 52.212-1, 52.212-4, and 52.212-5.

• Addenda to FAR 52.212-1 and 52.212-4 are included.

• A Justification for Other than Full and Open Competition (J&A) is attached (referenced in Section D).
