Statement of Work (SOW)

To establish a Blanket Purchase Agreement (BPA) for the purchase of medical repair parts and supplies on an as-needed basis for the Phoenix Indian Medical Center (PIMC) Biomedical Department. The goal is to ensure timely repairs, minimize equipment downtime, and maintain continuity of patient care by having readily available parts and supplies.

• Furnish all required medical parts and supplies on an as-needed basis.

• The agreement aims to streamline procurement, improve cost-effectiveness through volume pricing, and enhance efficiency in obtaining necessary materials.

• Pricing is based on current market rates for supplies, materials, and shipping costs and is fixed.

• Adjustments for unanticipated and material increases in supply or shipping costs due to inflation or market conditions may be mutually agreed upon through a formal amendment.

• Period of Performance: July 1, 2026, through June 30, 2031 (five one-year base periods).

• Delivery Location: Phoenix Indian Medical Center (PIMC), Engineering Department, 4212 North 16th Street, Phoenix, AZ 85016.

• A delivery ticket must accompany every delivery, including the delivery date and a description of items for accurate receipt and invoicing.

• Invoices must be submitted to the PIMC Finance Department (physical address and email provided) and include specific details such as contractor name, contract number, invoice number/date, cost/price, and dates of service.

• Contracting Officer Authority is reserved exclusively for negotiating changes.
