The government may also use information from CPARS, PPIRS, and FAPIIS.

  • Cover sheet with company name, CAGE code, and point of contact.

  • Price list (if applicable).
  • Three Past Performance References (using the attached questionnaire).
  • Active accounts in Procurement Integrated Enterprise Environment (PIEE) and System for Award Management (SAM).

Additional Notes

Vendors found to have technical and past performance merit will be issued a Master BPA, enabling them to bid on future requirements. If you possess a GSA contract, please note the number in your response.
