Statement of Requirements (SOW/PWS)

This document outlines the specific requirements for the delivery of medical and surgical supplies under a contract, detailing delivery terms, product specifications, and other operational necessities.

• Delivery Locations: Deliveries are to two Consolidated Mail Outpatient Pharmacy (CMOP) locations: Chelmsford, MA (for line items 1-3) and Tucson, AZ (for line item 4). All items are to be fulfilled in a one-time delivery.

• Shipping: Products must be shippable via USPS unless otherwise identified and approved by the Contracting Officer. The most cost-effective shipping method should be used for initial delivery.

• Product Identification: Shipments must reference the original VA obligation number for reconciliation. Tracking information must be emailed to designated points of contact upon shipment.

• Brand Name: All products are brand name only.

• Product Specifications:

  • Must include unit-of-use bar code labeling conforming to GS1-128 or HIBC standards, encoding the NDC.
  • Bar code printing must meet ANSI/ISO/IEC Quality Grade C or better.
  • Bar codes must be on the outside container/wrapper and immediate container, and on each cell of a blister pack.
  • Lot number and expiration date should be included on bar code labeling.
  • Products must have received an "A" therapeutic equivalence code from the FDA, unless all drugs in the family group are "B" rated.
  • Expiration dates must be no less than 12 months from the date of delivery.
  • Minimum bottle size is 100cc with a child safety cap, a cylindrical body, and a minimum of 5.75 inches circumference.

• Lot Numbers: A minimum of three (3) lot numbers of products are required. More than three (3) lot numbers require Contracting Officer approval. Products must be sorted and shipped with one (1) lot number per box.

• Deliveries to CMOP locations in Chelmsford, MA and Tucson, AZ.

• Acceptance: Acceptance of vendor-quoted NDC, MPN, or UPC is contingent on CMOP approval.

• Quoting: Vendors must specify the NDC/UPC used when quoting and indicate proposed alternates on the attached quote spreadsheet.

• Recalls: In case of a drug recall, vendors must immediately forward two (2) copies of the recall notification and pertinent information to specified VA offices.

• National Drug Codes & MPNs: Offerors must provide a distinct eleven-digit NDC or MPN for each product on the quote spreadsheet. Failure to do so will result in rejection.

• Availability: Offerors must provide a firm commitment of product availability and price 24 hours prior to purchase order.

• Other Requirements:

  • Packages ordered below 500 count require Unit of Use issue and a Safety/Child Proof cap.
  • Substitutions of pack sizes require written approval.
  • Changes deviating from requested items require Contracting Officer approval.
  • Partial deliveries require written approval.
  • Shipments must include a packing slip with specific identifying information.
  • All like products packaged and shipped together; breakable items require appropriate packaging.
  • Vendor must provide return authorization and pickup for damaged/incorrect products within one week of request.

Form / Attachment / Pricing Sheet

This document is a spreadsheet intended for bidders to provide pricing and capability information for specific medical supplies. It requires bidders to complete highlighted fields to indicate their ability to meet the requirements.

• Item Number: Sequential identifier for each product.
• Description: Details of the product, including manufacturer part numbers where applicable (e.g., ONE TOUCH VERIO (GLUCOSE) TEST STRIP).
• Quantity & Unit of Measure: Specifies the amount and unit for each item (e.g., 2400 EA).
• Manufacturer Part Number (MPN): The original equipment manufacturer's part number.
• OEM Name: The name of the original equipment manufacturer.
• Documentation & Compliance: Fields to confirm possession of direct OEM documentation and compliance with the Statement of Requirements (SOR).
• Business Information: Bidders must state their business size/classification and whether they hold a Federal Supply Schedule (FSS)/GSA/MS contract, including relevant contract and item numbers if applicable.

Sources Sought Notice

This notice is issued by the Department of Veterans Affairs, NCO 15 Consolidated Mail Outpatient Pharmacy (CMOP) Division for market research purposes. The government is seeking to identify potential qualified sources for the procurement of specific medical-surgical supplies. This is not a request for quotes, and no contract will be awarded based solely on responses to this notice.
