This is a Request for Quote (RFQ) for Luxury Vinyl Plank (LVP) flooring and associated supplies for Government Housing Renovations at the Chinle Comprehensive Health Care Facility (CCHCF). The procurement is a 100% Indian Small Business Economic Enterprise (ISBEE) set-aside.

• Luxury Vinyl Plank (LVP) Flooring:

  • For heavy/high traffic areas.
  • Minimum width: 7 inches, length: 47 inches, thickness: 2.5mm, wear layer: 20 mil.
  • 100% waterproof, scratch resistance, stain resistance, slip resistance.
  • 10-year minimum warranty.
  • Approved for glue-down system.
  • Wood look pattern.
  • Brand: Daltile, Product Series: Elixen.
  • Preferred Colors: Vintage Pine (EX32) and Driftwood (EX34).
  • Quantity: 20,022 square feet. Unit of issue to be indicated (Box, Carton, Bundle, etc.).

• Pressure Sensitive Adhesive:

  • Approved for LVP flooring, heavy-duty glue-down.
  • High moisture resistant.
  • Minimum 3-gallon containers (counted as 1 each).
  • Suitable for concrete, plywood, or other approved wood material.

• Shipping and Handling: All types included.

• Contract type will be firm-fixed-priced.

• Delivery: FOB Destination.

• Delivery ARO (After Receipt of Order): 60 days.

• Offers must be submitted via email to the Contracting Officer (Bennett Arvessto) by the due date: March 17, 2026, 1700 MDT.

• Quotes must include:

  • Solicitation number.
  • Offeror's name, address, telephone number.
  • Unique Entity Identifier (UEI) and EFT indicator.
  • Information for evaluation factors (see 52.212-2).

• Submission format requires three volumes:

  • Volume 1: Administrative Documents (signed SF-1449, IHS IEE Representation Form).
  • Volume 2: Technical Capability/Specifications (evidence of meeting specifications).
  • Volume 3: Price (detailed pricing aligned with SF-1449 and Section B, with narrative support).

• NO PRICING INFORMATION IN VOLUMES 1 or 2.

• All documentation must be in PDF format.

• The Government will evaluate offers based on the Lowest Price Technically Acceptable (LPTA) method.

• Offerors must hold prices firm for 60 calendar days.

• This acquisition is a 100% Indian Small Business Economic Enterprise (ISBEE) set-aside.

• The Buy Indian Act applies, requiring an Indian Economic Enterprise Representation Form.

• Brand name identified (Daltile, Elixen), but brand-name or equal descriptions are acceptable per attached specifications.
