Statement of Work (SOW)

To provide comprehensive pest control services for the Crow Service Unit (a 24-bed health facility) and its two satellite clinics in Crow Agency, Montana, to prevent and treat rodents, insects, and other vermin.

• Manage the total work effort for pest control services, including treatment and prevention.

• Utilize targeted methods such as habitat modification, biological/genetic/cultural control, mechanical/physical control, and judicious use of least hazardous pesticides.

• Implement planning, scheduling, warranty enforcement, and quality control.

• Provide adequate staffing with management expertise.

• Perform work with minimal interference to government business.

• Service specific departments including the Crow Hospital Kitchen, Entrance Areas (including garage bays), and the Maintenance Department. Additional departments may be added based on seasonal needs.

• Spray the 2nd floor perimeter to combat specific pests (Ants, Cockroaches, etc.) a minimum of two times per year.

• Provide insect lights with monthly fly trap replacement and annual bulb replacement for specified locations.

• All work must meet industry-accepted standards and all applicable federal, state, and local standards.

• Vendor must provide a Service Report to the COR within one week of services.

• Vendor must check in with the facilities manager upon arrival for each monthly visit.

• Period of Performance: Base Year (03/01/2026 to 02/28/2027) with four Option Years extending to 02/28/2031.

• Locations: Crow Service Unit (Crow Agency, MT), Pryor Health Clinic (Pryor, MT), and Lodge Grass Clinic (Lodge Grass, MT).

• Services are required once a month at each location.

• Vendor is responsible for obtaining access to buildings and facilities through the Contracting Officer or COR.

• Vendor must obtain all necessary appointments, licenses, and permits at their own expense.

• Vendor must comply with Federal Pesticide Regulations, Privacy Act, HIPAA, and 42 CFR Part 2.

• Indemnification and liability provisions related to the Terms of Service (TOS) are waived, except as authorized by law. Liability will be determined under the Federal Tort Claims Act or other governing federal authority.

• The TOS will be governed by U.S. laws, with Montana state laws applying in the absence of applicable federal law.
