Performance Work Statement (PWS)

This PWS outlines the requirements for providing support services to the Office of the Surgeon General (OTSG) / U.S. Army Medical Command (MEDCOM), Clinical Support Program Office (C-SPO), Patient Administration Division (PAD), and the Disability Evaluation System (DES) Program Office. The goal is to enhance the DES Program Office's capabilities in strategic analysis, planning, programming, and implementation of solutions to improve Army medical readiness.

The contractor shall provide qualified personnel to support the DES Program Office and the Medical Evaluation Board Tracking Office (MEBTO) in various locations, primarily Defense Health Headquarters (DHHQ) in Falls Church, Virginia, and Joint Base San Antonio (JBSA) in San Antonio, Texas. Key support areas include:

• Strategic, operational, and tactical analysis, planning, and implementation for DES solutions.

• Enhancing communication, capability building, evidence-based decision-making, fostering a healthy work environment, and patient advocacy.

• Generating management reports, tracking DES cases, and analyzing performance against metrics.

• Assisting in answering congressional inquiries regarding Medical Evaluation Board (MEB) activities.

• Participating in work groups to assess issues and provide recommendations.

• Supporting the drafting and refining of policies, operations orders, and other program documents.

• Providing data support and tracking for IDES cases, updating databases (e.g., SMS, TDRL-A), and presenting data in various forums.

• Providing administrative support for the DES Program Office medical advisor role, including reviewing fitness for duty cases and consulting on medical evidence.

• Supporting medical records management, including generating timeliness reports and assisting with legal corrections.

• Providing support to the MEBTO in San Antonio, including validating and tracking MEBs, coordinating with various Army components, and managing case files.

• Monthly Status Reports (MSRs) are required, detailing work accomplished, issues, deliverables, and travel.

• Quality Control Plan (QCP) must be maintained and submitted within 30 days of award.

• Monthly Quality Control Inspection Reports are required.

• Contractor manpower reporting is required annually via SAM.gov.

• Performance locations include DHHQ, Falls Church, Virginia, and JBSA, San Antonio, Texas.

• Services are to be provided between 0700 and 1700, Monday through Friday, excluding federal holidays.

• Personnel Security: Requires a favorably Interim Tier 2 Public Trust background check for access to federally controlled information systems. Common Access Card (CAC) may be required.

• Information Security: Strict compliance with HIPAA, Privacy Act, and DoD regulations for protecting patient and sensitive information.

• Training: Mandatory training includes DoD IA awareness, SHARP, HIPAA, OPSEC, and potentially MEDCOM-specific training.

• Key Personnel: A Project Manager (and Alternate) must be designated.

• Reporting: Various reports are required, including MSRs, QC reports, and specific data/metrics reports.

• Government Furnished Items: Government will provide office space, computers, software, access to systems (MODS, MHS GENESIS, etc.), and necessary supplies.

• Contractor Furnished Items: Contractor shall provide all other necessary items and non-personal services.

• Transition: A 30-day transition-in period is required, with a phase-in plan submitted within 5 days of award. A smooth transition-out is also required.

• Security: Compliance with OSHA, HSPD-12, and Army regulations for physical and information security is mandatory. Access to government facilities requires adherence to installation policies and potential background checks.

• Reporting: All contractor labor hours must be reported annually via SAM.gov.

• SHARP: Contractors must report any knowledge of sexual assault or harassment incidents occurring on government facilities within 24 hours.

The questionnaire seeks to understand common business practices, the feasibility of the requirement, and potential challenges. Specific questions cover:

• Small business qualification under NAICS code 562110.
• Relevant corporate experience in medical facilities.
• Anticipated challenges in performing the services.
• Innovative approaches to personnel recruitment and retention.
• Preferred teaming arrangements and joint venture details.
• Examples of similar past projects.
• Preferred pricing structures and incentive considerations.
• Anticipated challenges in preparing a price proposal.
• Acceptable timeframes for proposal submission.
• Current SAM.gov registration and services provided.
• General comments on the requirements.

This questionnaire is a critical tool for potential bidders to assess their capability and interest in the opportunity. Responses will help the government understand industry capabilities and refine the requirement. Bidders should carefully review the questions to determine if their firm possesses the necessary experience and to consider how they might approach the potential requirement. The information gathered will likely inform the structure and details of any future formal solicitation.

This document is an Experience Reporting Form that bidders must use to provide examples of past projects. It is intended to demonstrate the company's and its teaming partner's/subcontractor's experience with projects of a similar type and scope to the current opportunity.

• Bidders must provide up to three (3) project examples each for their company and their teaming partner/subcontractor.
• Projects must have been completed within the last 36 months, totaling 24 months of experience.
• For each project, the form requires details such as:
  • Name of the Firm
  • Contract Number, Title, and Location
  • Contract Type and Pricing Arrangement
  • Type of Work/Description
  • Relevance to the immediate acquisition
  • Role (Prime, Joint Venture, Subcontractor)
  • Contract Award Amount
  • Dates of Contract (Began and Completed)
  • Termination or Liquidated Damages information

This form is critical for bidders to demonstrate their qualifications and relevant past performance. Failure to provide complete and accurate information as requested in this form may impact the evaluation of the proposal.
