• Within ten calendar days of award or new employee selection, a list of employee details (name, address, SSN, DOB, work phone) must be provided.

• Excessive personnel turnover is defined as more than 3 occurrences within a rolling 12-month period.

• A maximum of three background investigations will be conducted at the Government's expense per 12-month period; additional investigations will be at the Contractor's expense.

• Important Note: References for subcontractors from the proposing offeror are not acceptable. Written references for subcontractors are encouraged.

Potential bidders who intend to use subcontractors or teaming partners must ensure these partners complete and submit this form. The prime offeror must include these completed consent forms with their past performance volume submission. Failure to submit these forms for major subcontractors/teaming partners may impact their consideration in the proposal. The reference list provides the government with contacts to verify the past performance of proposed subcontractors/teaming partners.

• Background Checks: Completed after award for building access.
• Carpet Cleaning: Total area is 11,323 SF, to be performed every 6 months. Previous cleaner subcontracted this task.
• Mopping/Waxing: Mopping area is 3830 SF; waxing is limited to the front entrance (550 SF).
• Restroom Finishes: Restrooms have solid surface countertops and stainless steel fixtures.
• Window Cleaning: Limited to interior windows only.
• Work Hours: Saturday work requires COR approval; standard hours are 6 PM - 6 AM, Monday-Friday.
• Floor Stripping/Polishing: This task is performed as needed, directed by the CO or COR.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for janitorial services at the Dixie National Forest Supervisor's Office and Cedar City Ranger District located at 820 North Main Street, Cedar City, UT 84721.

• Basic Services: Daily cleaning of approximately 16,000 square feet of office space, including reception/lobby, conference rooms, restrooms, offices, break rooms, and hallways. • Semi-Annual Tasks: Interior window washing, including wiping water spots from sills/frames and protecting adjacent surfaces. The COR may direct commercial carpet cleaning twice annually with approximately 21 calendar days' notice, to be performed on weekends.

• General Cleanliness: Areas must be free of dust, dirt, debris, carpet stains (excluding existing documented stains), trash, streaks, and smudges.

• Specific Areas: Desks and IT equipment are not to be cleaned. Horizontal and vertical surfaces, including furniture, counters, railings, etc., up to 70 inches high, must be free of dust and oil spots.

• Acceptable Quality Level (AQL): Generally 95% acceptability for services. Specific performance standards and surveillance methods are detailed in Section 9.0, including potential deductions for non-compliance.
