This document is Attachment 4, the Present and Past Performance Questionnaire (PPQ), for Solicitation HE125426RE008 concerning transportation services for the Vicenza, Italy Military Community. Bidders must complete the "To be completed by Offeror" section and then send the PPQ to their references (customers) for completion of the evaluation section. The completed PPQ should be returned via email to the specified Contract Specialist and Contracting Officer.

Offeror Information Section: Requires details such as Firm Name, N/CAGE Code, Address, Unique Entity ID, Phone Number, Email Address, Point of Contact, and information on work performed as Prime/Subcontractor/Joint Venture, including the percentage of work and prime contractor details if applicable.

Reference Information Section: Requires the Business Name, Address, Phone Number, Email Address, Point of Contact, Contract Number, Final Contract Value, Contract Type, and Period of Performance for the customer providing the reference.

Evaluation Section (to be completed by Offeror's customer): This section uses a rating key (P/U, S, G, VG, E, N) for evaluating the contractor's performance in areas such as:

• Transporting pre-kindergarten through high school grade children.
• Transporting special needs students with a Safety Aide.
• Providing Safety Attendant and/or Safety Aide services.
• Managing personnel and vehicles, including responsiveness to deficiencies.
• Responding to incidents and accidents.
• Maintaining vehicles.
• Providing, tracking, and reporting required training.
• Providing timely and accurate reports.
• Communication with stakeholders.
• Overall performance.

Explanation Section: Requires a brief description of the effort, including its relevancy to the RFP requirements, and explanations of any problems and resolutions.
