Statement of Work (SOW) / Performance Work Statement (PWS)

Provide construction services for the EHRM Installation of DDPO2 project (Project 695-26-702) at the Clement J. Zablocki VA Medical Center.

• Install 8 CAT6A duplex receptacle data drops and 8 duplex Emergency power outlets on the 2nd floor of Building 111 and the 1st floor of Building 146.

• Integrate CAT6A wiring into existing VA network switches and connect 120VAC power to existing electrical panels.

• Government-Furnished Equipment (GFE) including Oracle Connectivity Engines (OCE), Tracking Boards, and Tracking Board PCs will be provided for mounting.

• Locations and quantities for installations are detailed in attached drawings and spreadsheets.

• Extensive outage coordination is required before each installation.

• The project is located at Clement J. Zablocki VA Medical Center, 5000 West National Avenue, Milwaukee, WI.

• All work shall be completed within 90 calendar days after receiving Notice to Proceed (NTP).

• The overall project completion, including all phases, is within 120 days once NTP is issued.

• The project is divided into five phases:

  • Phase 1: Pre-construction (kick-off) Meeting.
  • Phase 2: Pre-Construction Period (NTP + 15 calendar days) for submitting plans (Logistics, Security, Phasing, Submittal Registry, Accident Prevention, Hazard Analyses, Schedule, Schedule of Values, Existing Conditions Survey, Infection Control, Quality Control, Environmental Protection, Document Submissions, Demolition Debris Management, Certificates/Licenses/Training, PIV badges).
  • Phase 3: Pre-Construction VA Review Period (NTP + 30 calendar days).
  • Phase 4: Construction Period (60 days, completed by NTP + 90).
  • Phase 5: Post-Substantial Completion Period (NTP + 120 calendar days) for final inspections, approvals, warranties, O&M manuals, as-built drawings, staff training, waste reports, and claims release.

• COR (Contracting Officer's Representative) review and approval are required for all submittals and commencement of work.

• Naturally occurring weather delays may require restarting construction once conditions permit.
