Scope of Work

Services must determine medical qualification for federal employment. The contractor must provide all necessary labor, facilities, equipment, and labs. Performance will occur at the contractor's offices, which must be located within driving distance of Grissom ARB, Indianapolis, Fort Wayne, South Bend, or Lafayette, Indiana. Key Requirements
Offerors must demonstrate the ability to meet all Statement of Work (SOW) requirements. Key personnel, specifically the Contract Physician, must be Board Certified in Occupational Health. Audiograms must be performed by a CAOHC-certified technician. Compliance with various standards is required, including NFPA, TIG 1582, OSHA, DoD, and Air Force Standards for Firefighters, and AFI 31-122 for Police Officers. Mandatory cybersecurity compliance: Contractors must implement NIST SP 800-171 requirements (DFARS 252.204-7012, 7019, 7020) and document assessment results in the Supplier Performance Risk System (SPRS). A valid self-assessment score is required, and a Plan of Actions & Milestones (POA&M) is necessary if a perfect score is not achieved. All contractors must be registered in SAM.gov. No foreign participation is allowed. The Service Contract Labor Standards (Wage Determination 2015-4809, Revision 33) apply.
