Scope of Work
The contractor will provide secure, on-site shredding services for confidential documents at Lyster Army Health Clinic (LAHC). This includes providing twenty-seven (27) 95-gallon containers with locks and 3" x 12" openings, which remain contractor property and will be dispersed throughout LAHC. Shredding will occur monthly, on the first Thursday of each month (1000-1500 hours), at the LAHC Warehouse loading dock parking lot. The shredder must be capable of handling staples, paper clips, gummed labels, cardstock, and glue-bound manuals. Documents to be shredded include HIPAA-protected patient records, PII, and prescription labels (documents only). Pill bottles are specifically excluded. Shredded material must be disposed of in accordance with HIPAA mandates, and a certificate of destruction must be provided via email to the COR upon completion. Contractor employees must be HIPAA certified with annual updates.
