Response Instructions
Interested firms must submit a Capability Statement (limited to 15 pages, 12-point font, excluding cover and administrative pages) via email to Jennifer Burke (jennifer.d.burke2.civ@health.mil). The email subject line must be "Stewardship and Sustainment for the Global Master Trainer Institute (GMTI) Program – SS Response."
Capability Statements should include:
- Company Cage Code, Unique Entity Identifier (UEI), name, address, point of contact, telephone number, email address, and company structure.
- Confirmation of System for Award Management (SAM) registration.
- Small Business Designation status (e.g., 8(a), HUBZone, SDVOSB, women-owned).
