Requested Information

Interested firms, especially Small Businesses, certified HUBZONE, 8(a), Woman-Owned Small Business, or Service-Disabled Veteran Owned Business contractors, should provide:

  1. Identification and verification of company small business status.
  2. Contractor's Unique Entity Identifier (UEI) and CAGE Code(s).
  3. Documentation from bonding company showing current single and aggregate performance and payment bond limits.
  4. Descriptions of Experience: Up to three (3) example projects (95% complete or completed in past 5 years) where the firm served as prime contractor. Projects must be of similar size and scope to the culvert replacement, including details on completion, scope, size, dollar value, design-bid/design-build, percentage of self-performed work, and number of subcontractors.
  5. Percentage of work to be self-performed on this project, in accordance with FAR 52.219-14 (at least 15% for general construction).
