Requested Information
Interested firms, especially Small Businesses, certified HUBZONE, 8(a), Woman-Owned Small Business, or Service-Disabled Veteran Owned Business contractors, should provide:
- Identification and verification of company small business status.
- Contractor's Unique Entity Identifier (UEI) and CAGE Code(s).
- Documentation from bonding company showing current single and aggregate performance and payment bond limits.
- Descriptions of Experience: Up to three (3) example projects (95% complete or completed in past 5 years) where the firm served as prime contractor. Projects must be of similar size and scope to the culvert replacement, including details on completion, scope, size, dollar value, design-bid/design-build, percentage of self-performed work, and number of subcontractors.
- Percentage of work to be self-performed on this project, in accordance with FAR 52.219-14 (at least 15% for general construction).
