• Role: Selection of the offeror's role (Prime Contractor, Subcontractor, Joint Venture, or Other).
• Project Description: A detailed description of the project and its relevancy to the current RFP requirements, including unique features, square footage, and construction methods.
• Self-Performed Work: A description of the work the offeror's firm performed directly.
• Other Information: A section for any additional relevant details.

Bidders must complete this form to detail their experience on relevant projects. This information will likely be used by the government to evaluate the offeror's qualifications and capability to perform the work described in the solicitation. Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for road maintenance activities within the Green Mountain National Forest.

• Aggregate Surface Reconditioning: Blading and shaping roadbed width to achieve uniform, consistent cross slopes (crowned or unidirectional) and smooth surfaces. This includes reclaiming aggregate material, avoiding the creation of secondary ditches or high shoulders, and removing unsuitable material. Specific requirements are detailed for different road levels (Level 2, Level 3 & 4).

• Culvert Cleaning: Removal of debris, stones, and slide materials from culvert catch basins, inlets, outlets, and associated cut slopes to ensure uninterrupted drainage flow. Waste materials must be disposed of properly.

• Ditch Reconditioning: Re-establishing and maintaining functional roadway ditches to ensure continuous, free-flowing water. This involves reshaping ditches to match existing grades and cross-sections, avoiding undercutting, and proper disposal of unsuitable materials.

• Road surfaces must be uniform, consistent to grade, and free from corrugations, potholes, and ruts.

• Stones protruding more than specified heights (4 inches for Level 2, 2 inches for Level 3 & 4) must be removed.

• Dips must have a minimum drainage flow depth of nine (9) inches.

• Drainage run-outs must be provided at specified intervals and lengths.

• Sediment prevention measures are required for ditches and locations near active streams.
