Performance Work Statement (PWS)

To procure residential programming and supportive services for homeless Veterans in Eureka/Humboldt County, CA, as part of the Health Care for Homeless Veterans Contract Emergency Residential Services Program (HCHV CERS).

• Provide community-based, short-term residential services (30-90 days, extendable) for up to 15 homeless Veterans.

• Offer supportive services including mental health stabilization, substance use disorder treatment, independent living skills, vocational assistance, and income support.

• Facilitate transition to permanent housing.

• Ensure safe and secure housing available 24/7, including meals, laundry, hygiene kits, and personal storage.

• Provide case management, care coordination, and linkage to VA and community resources.

• Adhere to specific admission practices, not denying entry based on abstinence, legal history, income, etc.

• Securely store and monitor medications.

• Maintain program integrity and safety with clear rules regarding violence, alcohol/drugs, and sexual activity.

• Maintain a minimum occupancy rate of 80% (90-100% preferred).

• Achieve a target of 55% or more Veterans exiting to permanent housing (FY26 target).

• Limit negative exits (due to rule violations) to 20% or less (FY26 target).

• Submit various reports including referral reports, daily census, critical incident reports, admission/discharge reports, and quarterly performance reports.

• Location: Eureka / County of Humboldt, CA.

• Contract Duration: Up to five years (one 12-month Base Year and four additional 12-month Ordering Periods).

• Facility must meet safety, sanitation, ADA, and fire safety codes (NFPA 101).

• Contractor must provide 24-hour staffing, including a program manager and a credentialed clinician.

• Staff must be trained in motivational interviewing, de-escalation, and other relevant areas.

• Strict adherence to VA information security, privacy, and records management policies is required.

• Contractor must participate in HMIS data reporting.

• Facilities will be subject to regular VA inspections.

This is a Sources Sought Announcement for market research purposes only. The Department of Veterans Affairs (VA) Network Contracting Office (NCO 21) is seeking information from vendors capable of providing Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) for the San Francisco Veteran Affairs Health Care System (SFVAHCS) in Humboldt County/Eureka, CA. No solicitation is currently available, and this announcement is not a commitment to award a contract.

• The service involves providing 15 beds per day to homeless veterans on a per diem basis, in accordance with a draft Performance Work Statement (PWS).

• The projected period of performance spans five ordering periods, from 09/01/2026 to 08/31/2031.

• The applicable NAICS Code is 624221 (Temporary Shelter) with a small business size standard of $13.5 Million.

• For small businesses, including SDVOSB and VOSB, there is a Limitations on Subcontracting requirement (FAR 52.219-14 and FAR 52.219-27), requiring at least 50% of the cost of contract performance incurred for personnel to be expended for employees of the concern.

• SDVOSB and VOSB firms must be registered in SAM and SBA verified.

• Contractors must have a current registration in the System for Award Management (SAM).

• All information submitted is preliminary and subject to modification.

• The Government will not pay for any information submitted.
