This is a combined synopsis/solicitation (RFQ) for a full-service maintenance agreement (SMA) for a Lumenis Smart Selecta Duet Laser (GAS910000LED, SN: 10346) used in Ophthalmology Services at the VA Texas Valley Coastal Bend Healthcare System in Harlingen, TX. The government is seeking a contractor to provide all necessary parts, labor, tools, and travel for scheduled preventative maintenance and emergency repairs.

• Full-service maintenance and repairs for the specified Lumenis laser.

• Services must adhere to Original Equipment Manufacturer (OEM) scheduled maintenance requirements and specifications.

• Work to be performed by certified and qualified Field Service Technicians with OEM training.

• Provide technical support/help desk Monday-Friday, 8:00 AM - 5:00 PM CST.

• Phone response within 60 minutes, on-site response within 24 hours.

• Achieve 95% uptime.

• Parts must be OEM and new.

• Contractor must provide a written service report after each visit.

• Contract Type: Firm-fixed price.
