Statement of Work (SOW)

To define the tasks, responsibilities, and deliverables for debris removal services at the designated MODOC area, ensuring the site is cleared of debris, waste material, and obstructions in a safe, timely, and environmentally responsible manner.

• Remove down trees from three buildings (bathroom, picnic, storage).

• Remove down trees on the grounds that obstruct park usage.

• Mow grounds and weed eat around buildings, tables, walkways, and bushes.

• Edge walkways as needed.

• Blow clean all walkways, driving, and dock areas.

• Remove trees and debris into dumpsters.

• Provide all labor, equipment, tools, and transportation.

• Performance Period: One-time contract, completed within sixty (60) days from the date of award.

• Place of Performance: Augusta VA Healthcare Medical Center, 1 Freedom Way, Augusta GA 30904.

• Acceptable working hours are 8:00 am to 4:00 pm, Monday through Friday, excluding federal holidays.

• Contract Type: Firm-Fixed-Price.

• Changes to the SOW must be authorized and approved in writing by the Contracting Officer (CO).

• Payment will be made upon inspection of workmanship by VA personnel and receipt of a properly prepared invoice submitted electronically through OB-10.

• Invoices must include: Invoice Number and Date, Contractor's Name and Address, Accurate Purchase Order Number, Supply or Service provided, and Total Amount Due.
