Evaluation

Submissions must include the signed Quotation Pricing Sheet (Enclosure C), Annual Representations and Certifications (Enclosure B), and necessary tax forms (W-9 or W-8). Vendors must be registered in SAM.gov with a valid Unique Entity ID (UEI).
