• CMMC Requirement: Offerors must possess a valid CMMC Level 1 (Self-Assessment) certification at the time of offer submission and for the duration of any resulting contract.

• This acquisition is set aside for Small Business.

• OPSEC Plan is not required.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide one (1) fully operational 10-ton capacity forklift for rental to Pearl Harbor Naval Shipyard & IMF (PHNSY & IMF) to support day-to-day activities.

• Provide one (1) 10-ton capacity forklift, delivered to PHNSY & IMF.

• Forklift to be operated exclusively by Government licensed and authorized personnel.

• Contractor is responsible for all maintenance, repairs, and support to ensure continuous safe operation.

• Forklift specifications: 20,000 lb minimum capacity, diesel fuel only, pneumatic tires.

• Equipment must be delivered in good working order, free of defects, and fully operational.

• All safety features (horns, lights, back-up alarms) must be functional.

• Contractor must provide current inspection and maintenance records.

• Delivery to PHNSY & IMF prior to the contract base period start.

• Removal within two (2) working days after the final contract performance period.

• Equipment Availability: 95% availability per month. Calculated as (Total Scheduled Hours - Downtime Hours) / Total Scheduled Hours.

• Response to Equipment Failure: Provide a replacement unit within 24 hours of notification. No more than one late replacement per 6-month period.

• Scheduled Maintenance Compliance: 100% on-time completion of scheduled maintenance events per COR-approved schedule.

• Place of Performance: PHNSY & IMF, JBPHH.

• Period of Performance: Base Period: 12 months from date of delivery. Option Period: One (1) 6-month option period.

• Applicable Documents: Compliance with OSHA standards (29 CFR 1910.178), manufacturer safety and maintenance requirements, and state/local transportation regulations.

• Pre-use Inspections: Government will conduct detailed pre-use inspections covering tires, rims, engine oil, fluids, battery, fuel system, noises, lights, horn, hoist, tilt, side shift, transmission, directional controls, brakes, gauges, fire extinguisher, operator restraint system, forks, fork positioning, safety chains, overhead guards, ground straps/tires, structural integrity, and markings.

• Maintenance: Contractor to provide manufacturer-recommended preventive and unscheduled maintenance. Develop and submit a Preventative Maintenance Schedule for COR approval. Respond to maintenance requests within 2 hours. Repairs must be completed within 8 hours; if not, provide a comparable replacement within 24 hours.

• Tire Damage: Contractor responsible for normal wear and tear, defects, or equipment failure. Government responsible for damage from misuse, debris, or Government personnel.

• Equipment Damage: Government responsible for damage by licensed operators. Contractor responsible for damage from equipment failure, improper maintenance, or normal wear and tear.

• Contractor Qualifications: Must ensure necessary employee expertise, comply with Safety and Health, environmental guidelines, and security requirements (background check, DBIDS registration, gate pass coordination).

• Government Furnished: None provided. Contractor may utilize existing electrical/air outlets but is responsible for any resulting damage.

• Hours of Operation: PHNSY & IMF operates 24 hours/day; administrative staff work 0630-1500 Mon-Fri. Holiday observance follows Federal guidelines.
