Scope of Work
PHNSY & IMF requires a fully operational 10-ton capacity forklift (minimum 20,000 lb, diesel fuel, pneumatic tires) for a base period of 12 months and one 6-month option period. The contractor will be responsible for providing the forklift, all associated labor, equipment, tools, materials, maintenance, and repairs to ensure continuous safe operation and 95% monthly availability. The forklift will be operated by Government-licensed personnel.

Submission Requirements
Offerors must submit:
- Completed SF 1449.
- A Technical Capability Statement (max 5 pages) demonstrating understanding of the PWS, including forklift specifications/brochure, a subcontracting statement (if applicable), and certification of delivery by the POP start date.
- Past Performance information (max 3 pages) detailing 2-3 similar contracts within the last two years.
- CMMC Level 1 (Self-Assessment) certification is mandatory at the time of offer submission and for the contract duration.
- All firms must be registered in SAM.gov by the award date.
- Offers must be submitted via email only to the listed POCs. The email size limit is 35 MB per email.
Evaluation and Award
Award will be made to the lowest priced technically acceptable offeror with an acceptable past performance rating. Technical capability will be evaluated first, followed by past performance, and then price.
Compliance & Amendments
- Compliance with environmental protection guidelines, safety and health work practices, and Operations Security (OPSEC) requirements is mandatory. An OPSEC Plan is not required.
