Statement of Work (SOW) / Performance Work Statement (PWS)

To provide all management, tools, supplies, equipment, and labor necessary to provide for and repair a dock plate and leveler, enabling safe loading/downloading of commercial/military vehicles and equipment. The work is located on the east side of building 90639, Hurlburt Field, FL.

• Inspect, quote, and repair THINMAN leveler (Serial number 20jc720058C) and the hydraulic docking plate.

• Provide mounting stations, equipment, and materials for leveler and dock functionality.

• Inspect existing pistons/velocity fuses and flow control valves; install new weather strips.

• Provide professional installation and hydraulic services to connect units to existing electrical systems.

• Install/unpack new items, perform assembly per manufacturer's instructions, and remove/dispose of packing material off-base.

• Vendor must have documented history of installing/performing maintenance on this type of equipment.

• A license to perform the repair will be presented before award of contract.

• Installation shall not exceed 7 calendar days from project start.

• Location: East side of building 90639, Hurlburt Field, FL.

• Hours of Operation: MON-FRI 0630 to 1600, excluding federal holidays. Extended hours may be approved by the Contracting Officer for timely completion at no additional cost. Installation times require coordination and approval by 1SOLRS personnel.

• Holidays: Service is not required on New Year's Day, MLK Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Holiday observance follows specific rules if falling on a weekend.

• Base Access: Contractor personnel require base identification, special clearances, and vehicle passes. An employee listing must be submitted to the Contracting Officer at least 3 days prior to performance start and one day prior to any changes. Employees with active warrants or felonies will be denied access. Two forms of identification are required for base access. If driving on base, a valid driver's license, vehicle registration, and proof of insurance are needed. Commercial Gate Control Center closes at 1800.

• Weapons/Firearms: Prohibited on base for contractor employees.

• Driving: Compliance with base traffic regulations is required. Employees must possess a valid driver's license for the type of vehicle operated.

• Government Furnished Property: None will be provided.
