Statement of Requirements (SOR)

This document outlines the requirements for products supplied under a resulting purchase order, focusing on delivery, product specifications, and contractor obligations.

• Delivery Terms: Government reserves the right to cancel undelivered quantities. Products must be shippable via USPS unless otherwise identified. Shipments must reference the VA obligation number. Deliveries to be coordinated with receiving CMOP facilities during operational hours. Tracking information to be emailed to Joseph.sousa2@va.gov upon shipment.

• Additional Terms: Brand Name Only/Dispense as Written is an option. Vendor-quoted NDC, UPC, or MPN requires CMOP approval prior to purchase order. Alternate terms require Contracting Officer review and acceptance.

• Bar Coding: All pharmaceutical/medical surgical products require unit-of-use bar code labeling conforming to GS1-128 or HIBCC standards. Bar codes must encode NDC and meet ANSI/ISO/IEC Quality Grade C or better. Bar codes must be on the outside container and immediate container (if not visible through overwrap), and on each cell of a blister pack. Lot number and expiration date should also be included.

• Therapeutic Equivalence: Only products with an FDA therapeutic equivalence code of "A" will be considered, unless all drugs in the family are "B" rated.

• Recalls: Vendors must immediately notify specific points of contact for any drug or medical surgical product recalls or FDA withdrawal of approval.

• National Drug Codes/Manufacture Product Number: Offerors must provide a separate and distinct eleven-digit NDC or MPN for each product.

• Firm Commitment: Offerors must provide a firm commitment of product availability and price 24 hours prior to purchase order.

• Other Requirements:

  • Expiration dates must be no less than 12 months from delivery.
  • Substitutions of pack sizes, changes deviating from requested items, and partial deliveries require Contracting Officer approval in writing.
  • Minimal lot numbers are required; more than three (3) lot numbers require written approval. Products must be sorted and shipped with one lot number per box.
  • Shipments must include a packing slip with specific identifying information (Vendor name/address, VA PO Number, VA Obligation Number, NDC, PRODUCTS, LINE ITEM NUMBERS) and a point of contact for discrepancies.
  • Like products must be packaged and shipped together. Breakable items require appropriate packaging. Quantities less than case size may be shipped in repack boxes if clearly marked.
  • Vendors must provide a return authorization and pickup for damaged or incorrect products within one week of request.

• Quotes failing to provide NDC/MPN by the solicitation closing date will be rejected.

• A firm commitment of availability and price is required 24 hours prior to purchase order.
