Scope of Work

The requirement covers a broad range of medical services. For Employee Services (NPD), this includes fitness for duty evaluations, return to work reviews, medical documentation reviews, medical and psychiatric consultations, and administration of the Drug-Free Workplace Program (DFWP), encompassing drug testing and Medical Review Officer (MRO) services. For Pre-Employment Services (CBP HC), the contractor will provide pre-employment medical examinations, fitness testing (PFT-1), drug test specimen collection and analysis, medical and mental health consultations, and MRO services for candidates. Services are required nationwide and at select international locations, with the contractor maintaining a network of facilities within a 60-mile radius of employee or candidate locations (exceptions for overseas/rural areas). The contract supports high-volume, time-sensitive processing, with historical annual volumes exceeding 40,000 pre-employment medical exams, 50,000 fitness tests, and 45,000 drug tests.

Contract Details

This is a Single-Award IDIQ contract with a Firm Fixed Unit Price structure for task orders. The Period of Performance is 60 months, consisting of five 12-month ordering periods, tentatively from August 1, 2026, to July 31, 2031. The estimated IDIQ maximum value is between $400,000,000 and $600,000,000. This acquisition is UNRESTRICTED. Contractor personnel must undergo background investigations (T4 or T5) and comply with DHS/CBP security, privacy, and IT requirements, assuming a FedRAMP Moderate baseline for IT security.

Proposal & Evaluation

The solicitation follows a Two-Phase Process. Phase I requires submission of a written technical/management approach, past performance, and a prior experience questionnaire. Phase II involves an oral presentation, a Small Business Utilization Plan, and a cost/price proposal. Proposals must be submitted electronically, with multiple emails allowed for complete volumes. Evaluation factors include Technical & Management Approach, Prior Experience, Past Performance, Oral Presentation, Small Business Utilization, and Cost/Price. Non-price factors combined are significantly more important than cost/price, and award will be made on a best value trade-off basis.

Key Requirements & Notes

Offerors must provide a detailed transition-in plan with their proposal (can be an appendix) and a draft Quality Control Plan within 5 days of award. Key personnel include Program Managers, Laboratory Director, Medical Review Officer, and various Reviewing Physician Consultants. The contractor must maintain a secure, web-based data system with real-time dashboards accessible to CBP, updated at least daily. Electronic Chain of Custody Forms (eCCF) are accepted with phased implementation. Specific Service Level Agreements (SLAs) for drug testing include dispatch within 1 hour and collection within 8 hours for post-accident testing, and response within 30 minutes with location within 1 hour for reasonable suspicion. Notification of an employee failing to appear for a drug test must be made to the COR within 2 hours. All offerors must be registered in SAM.gov.
