- Required Information: Submissions must include completed blocks 17a, 30a, 30b, 30c of Standard Form 1449, prices on Page 3, Unique Entity Identification (UEI) number, a statement of SAM registration, a statement of small business status under NAICS 336611, an Organizational Conflict of Interest statement, a response to evaluation factors (FAR 1352.213-70), proof of required insurance coverage, and a statement of compliance with the Sexual Harassment Prevention and Response Policy.
