Statement of Work (SOW) / Performance Work Statement (PWS)

To provide comprehensive pest control and termite treatment services for USPFO-PR, Building 541-Warehouse at Fort Buchanan, PR. The goal is to eradicate, suppress, and prevent pest populations, including termites, rodents, and other hazardous pests, to mitigate health risks, prevent property damage, and maintain a pest-free environment.

• Services: Provide all personnel, equipment, materials, and supervision for pest control and termite treatment.

• Location: USPFO-PR, Building 541-Warehouse, Borinqueneer Road, Fort Buchanan, PR 00934.

• Facility Size: Approximately 42,000 square feet with an 820 linear feet perimeter.

• Pests Targeted: Subterranean and drywood termites, ants, bees, wasps, mosquitoes, flies, ticks, fleas, cockroaches, mice, rats, snakes, lizards, crickets, earwigs, silverfish, spiders, centipedes, snails, and slugs.

• Methods: Employ chemical and non-chemical control techniques, prioritizing environmentally sound options. Includes exterior/interior treatments, soil injection, trenching, and colony elimination.

• Integrated Pest Management (IPM): Services must align with the U.S. Army Garrison Fort Buchanan IPM Plan. A detailed Pest Management Control Plan is required, specifying treatment areas, methods, approved pesticides (from Fort Buchanan's Pesticide Use List), safety procedures, and trap placement.

• Post-Treatment: Seal and clean all treated areas to restore aesthetics and leave the site clean.

• Pest Management Plan (PMP): Must be submitted within 30 days of contract award and meet all PWS and IPM Plan requirements.

• Service Completion: 95% of scheduled services must be completed.

• Safety: Pest management must be conducted with minimal environmental impact and risk of contamination. Safety plans are required.

• Period of Performance (PoP): Starts no more than 45 days after contract award and IPM approval.

• Work Hours: Monday through Friday, 0800 – 1530, excluding recognized US holidays or facility closures.

• Place of Performance: USPFO-PR, Building 541-Warehouse, Fort Buchanan, PR 00934.

• Licensing & Certification: Contractor must provide proof of commercial Pesticide Business License, Puerto Rico pesticide applicator certification/licensing for all personnel applying pesticides, and acknowledge minimum work categories (3, 4, 6, 7, 8C, 8E, 9).

• Pesticides: Use only EPA, DoD AFPMB, National Guard, and Fort Buchanan approved pesticides. Pesticides cannot be stored, mixed, or disposed of on-site.

• Insurance: Proof of liability insurance is required.

• Installation Access: Contractor personnel must comply with Fort Buchanan's access control and security procedures, including background checks. Vehicles are subject to search, and traffic violations may result in loss of driving privileges.

• Deliverables: Pest Management Plan, commercial licenses, insurance proof, pesticide lists, and applicator certifications are required prior to commencing services.
