Statement of Work (SOW) / Performance Work Statement (PWS)

To provide portable toilet rental and service for Columbus AFB, MS, including pumping out holding tanks.

• Provide personnel, tools, labor, transportation, equipment, and supervision for portable toilet delivery and service.

• Pump out four (4) 1,000-gallon holding tanks on Columbus AFB.

• Pump out two (2) 150-gallon holding tanks at Auxiliary Field OLF Alpha in Shuqualak, MS.

• Contractor-owned toilets must be self-contained, clean, and sanitary.

• Units must have hand sanitizer dispensers.

• Units must be maintained in good working condition; non-repairable units must be replaced within 24 hours.

• Long Term: Deliver 8 units to various locations on Columbus AFB within one workday after the contract start date. Units will be staked down.

• Short Term: Be prepared to provide up to 10 extra units for special events or emergencies within 2 hours of notification. This includes delivery, setup, servicing, and restocking.

• Provide an emergency phone number for after-hours contact.

• Relocate units on base at no cost to the government upon request.

• Furnish Material Safety Data Sheets (MSDS) for all cleaning and disinfecting compounds.

• Certify that all chemicals are non-toxic and non-corrosive.

• GTR will provide MSDS to 14 CES/CEAN for review before products are brought on base.

• Monthly HAZMAT inventories are required, including solvents, paints, degreasers, ozone-depleting substances, and oils.

• Monthly HAZMAT inventory must be submitted to the GTR within 5 business days of the preceding month.

• Long-term units: Service once monthly between 7:00 AM and 4:30 PM, Monday-Friday.

• Service includes waste removal and disposal (per Federal, state, and local laws), trash removal, unit cleaning (inside and out), repairs, refilling consumables (toilet paper, hand cleaner), and chemical restocking.

• Services rendered will be recorded on a label inside each unit.

• Contractor assumes liability for damages due to accidents or weather.

• Repair or replace units unsuitable for use within 24 hours of notification.

• Pump out 1,000-gallon tanks 12 times per contract year.
